Federally and non-federally regulated drug testing



There is federally and non-federally regulated drug of abuse testing. First one started by Ronald Reagan executive order, requiring all specified occupations federal employees refrain from consuming forbidden narcotic substances.

Special institution called Substance Abuse and Mental Health Services Administration or SAMHSA regulates narcotic testing guidelines and processes (formerly National Institute on Drug Abuse or NIDA established federal legislation standards).

It is required that companies, who use professional drivers, specified safety sensitive transportation, oil and gas related occupations workers, and some other particular federal employers check their staff for presence of certain types of drugs. These test classes were established decades ago.

They include five specific drugs of abuse groups and basically are not topical for current drug usage patterns. For instance, SAMHSA / DOT tests do not include semi-synthetic opioids, such as oxymorphone, oxycodone, hydromorphone, hydrocodone, hydromorphone, and very popular nowadays prescription medications.


According to federal SAMHSA/NIDA guidelines laboratories report quantitative results for the "NIDA-5" official tests. At the same time many drug testing private laboratories and on-site employer’s testing facilities offer wider and "more appropriate" set of drug screens which are more reflective of current drug consumption patterns.

These screens include prescription medications and pain killers such as benzodiazepines (Valium, Xanax, Klonopin, Restoril), Oxycodone (Oxycontin, Percocet), Hydrocodone (Vicodin), Hydromorphone, Oxymorphone and barbiturates in other drug panels.

"Panel" is a predetermined list of tests to run. The confirmation tests (usually LC/MS/MS or GC/MS) are sensitive to the difference between chemically similar drugs formulations such as methamphetamine and methylenedioxymethamphetamine (MDMA or ecstasy).

If in the sample there is absence of detectable amounts of methamphetamine, the laboratory may either admit the sample as negative or recognize it as positive for MDMA, if MDMA is included in the panel as something to be tested for.
